Based on record reviews, the licensee did not comply with the section cited above in two out of two infants sleeping log documentations which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee must ensure that the 15 minute sleep log is documented. Moving forward the licensee must document sleep checks for all infants under 24 months, and all checks must be dated. A return visit is required to ensure the deficiency has been corrected.

Based on record review and interview, the licensee did not comply with the section cited above in keeping up to date records for helper of complete immunization records or have them available for review during the inspection MMR test which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee must ensure all staff has up to date immunization records before they start working.The immunization records of the licensees helpers should also be included in the personnel files and have them available for review. A return visit is required to ensure the deficiency has been corrected.

Based on the record review, the facility did not comply with the abovementioned section. The licensee’s helper has not taken the Pediatric First Aid training as required of a large licensed childcare home, which poses/poses a potential health, safety, or personal rights risk to persons in care.

Resolution: The Licensee helper must enroll in a Pediatric First Aid and CPR course and send a copy of the registration to the licensing office by 12/22/2023. Once the course is completed, send the current certification to the licensing office.

Based on observation, interview, and record review, the licensee’s helper did not have proof of complete immunization records or have them available for review during the inspection. This poses a potential safety hazard to children in care.

Resolution: The Licensee must ensure that all personnel's immunization records are on file and available for review. The Licensee states she will have the helper’s immunization records available by December 22, 2023. A return visit is necessary to verify the documents.

Based on record review, both Licensees did not comply with the section cited above. Both Licensee and helper have not completed their Mandated reporter training, which poses/posed a potential health, safety, or personal rights risk to persons in care

Resolution: The Licensee states she will ensure that all current or new staff members complete the mandated reporter training every two years. The Licensee says she will have all staff complete the training by June 20. A return visit is necessary.

Based on record review, the Licensee did not comply with the section cited above. The Licensee and helper did not have immunization records, such as measles, pertussis, or TB clearance on file for review, which poses/posed a potential health, safety, or personal rights risk for persons in care.

Resolution: Licensees must ensure that immunization records for herself and the helper are kept at the facility and available for review upon request. The Licensee states she will request the record from the doctor and have it available by June 20, 2022.
