Based on observation, interview, and record review, the licensee did not comply with the section cited above in as there is an uncleared adult present in the home, which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Facility will have uncleared adult resolve fingerpirnt clearance. Facility will not allow uncleared adult to be present in the facility until fingerprints are cleared.
Based on observation, interview, and record review, the licensee did not comply with the section cited above as there are a total of five infant (under two years old) children present which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee will reduce the number of infants present to a total of four infants present at any given time. Licensee will submit an updated Children's Roster and schedules of infant children to LPA Tapia-Mandujano by the end of the day 1/12/21.
