Based on interview and record review, the licensee did not comply with the section cited above. The facility does not have child care insurance or has not completed form LIC 282, which poses a potential health, safety or personal rights risk to children in care.
Resolution: The licensee said she will purchase child care insurance and submit proof to the department by the POC due date.
Based on observation, the licensee did not comply with the section cited above. Bedroom # 2 was being used without prior approval from the department, which poses a potential health, safety or personal rights risk to children in care.
Resolution: During today's visit, the licensee provided a new facility sketch, and the LPA completed a health and safety check of Bedroom #2. The deficiency will be cleared during this visit.
Based on record review and interview, the licensee did not comply with the section cited above in that the licensee and 2 helpers did the non-EMSA certified CPR and First Aid training in March 2024, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: The Licensee scheduled the CPR and First Aid training on 8/20/2024 for all licensee and 2 helpers. The valid certificates would be sent to the Licensing Office by 8/30/2024.
Based on (observation), the licensee did not comply with the section cited above in licensee had 7 infants in care which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to disenroll 4 infants, have all parents read the Licensing Report dated 10/11/23, and have parents sign LIC 9224 Acknowledgement of Receipt of Licensing Report
Based on (record review), the licensee did not comply with the section cited above in that 7 out of 7 infant's sleep has not been logged which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to log sleep and email to LPA by the above stated date.
Based on (record review), the licensee did not comply with the section cited above in all infants under 12 months do not have sleep logs which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to collect sleep logs for infants under 2 and email to LPA by the above stated date.
