Based on the record review, the Center did not comply with the section cited above. it was revealed that the Director had deliberately altered her Mandated Reporter training certificate by changing the name of the certificate holder to reflect her own. This discrepancy was uncovered when the QR code on the certificate was scanned, which indicated that the certificate rightfully belonged to one of her teachers. This poses an immediate health, safety, or personal rights risk to persons in care.

Based on observation, an uncleared fingerprint person was present while the children were in care, which poses an immediate health, safety or personal rights risk to persons in care.

Based on interviews and records reviews, it was revealed that the center did not comply with the above section. The Center failed to conduct lead testing of its water used for consumption before the January 1, 2023 deadline. Failing to complete the drinking water test for lead contamination levels could pose potential health and safety risks to the children in care.

Based on record review and interview, the Center did not comply with the section cited above, as the Center does not have proof that fire drills are being conducted and documented at least once every six months. Not conducting fire drills could pose/posed a potential health, safety or personal rights risk to children in care.

Based on observations, LPAs confirmed that the center has a container of Lysol wipes that was stored on top of the children's toilet tank and was accessible to children.

Based on the record review, the Center did not comply with the abovementioned section. S1 has not completed their Mandated reporter training, which poses/posed a potential health, safety, or personal rights risk to persons in care.

Records such as measles, pertussis, Influenza, and TB clearance were missing or not available for review, which poses/poses a potential health, safety, or personal rights risk for persons in care.

Based on the record review, the center did not comply with the abovementioned section. The director did not have a valid mandated reporter training certificate. Additionally, it was discovered that the director had falsified the certificate by altering and editing her name on the certificate. The QR code verified that the certificate belonged to her staff member, Manlian Li.
