Based on observation, interview and record review, the licensee did not comply with the section cited above in 4 out of 4 infants' 15-minute sleeping chekcing record was not found at the facility, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The Licensee should conduct the 15-minute sleeping check and record the checking. The 15-minute sleeping checking records should be sent to the Licensing Office on or before the Plan of Correction due date, 2/19/2025.

Based on (observation), the licensee did not comply with the section cited above in dried dog poop was found in two different places in the back yard where children play which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee removed dried poop during visit. Licensee will put a gate up to keep family dog away from children's play area. in two weeks.

Based on (observation), the licensee did not comply with the section cited above in [count] out of licensee stored Draino in the daycare area accessible to day care children which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to store the draino in an off-limits area (the garage) or lock cabinet beneath sink. Licensee removed the Draino to an off-limits area durings visit.
