Based on observation, the licensee did not comply with the section cited above that LPA observed there was a blanket covering an infant and a cloth covered the edge of the crib. The Licensee removed them immediately, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: The Licensee should delveop a plan for Safe Sleep regulations compliance. The Plan should be submitted to the Licensing Office on or before the POC due date.
Based on observation, interview and record review, the licensee did not comply with the section cited above that LPA observed 5 infants in care during the visit. One of the infants would reach age of 2 on 1/18/2026, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: The Licensee should develop a plan for capacity and ratio compliance. The plan shoud be sent to the Licensing Office on or before the due day. A Plan of Correction visit would be conducted.
