Based on interview and record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Nap Logs are to be kept for infants 24 months and younger. The licensee shall physically check on the infants every 15 minutes and document the following: Labor breathing, sign of distress which includes but is not limited to flushed skin color, increase body temperature and restlessness. A follow up visit will be conducted.

Resolution: Licensee will have parent fill out LIC 9227. Proof of form will be emailed to LPA by due date 09/11/2024.
