Based on interview, the licensee did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care. LPAs observed 13 dropped off with 3 staff members going in to the facility. Upon entering into the facility and during the inspection LPAs were able to verified 2 staff members and 12 children. LPAs requested to inspect the off limit areas and Ms. Chen said she does not have the keys to open the rooms.

Resolution: Follow up inspection will be conducted.

LPAs observed 13 children being dropped off at the facility and observed 12 children inside the facility (2 infants, 10 preschool children). Based on record reviewed provided by Ms. Chen, no TK and/or elementary children records.

LPA and the licensee, Helen entered the facility together and LPA observed one helper supervising 11 children. Based on observation, the licensee did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: The licensee arrived at the facility and the deficiency cited is corrected.

LPA waited outside for 20 mins, helper did not answer the door. A civil penalty of $250 was issued today.

Resolution: The licensee, Helen arrived at the facility and let LPA inpsect the facility.

LPA introduced herself with proper identification, the helper answered the door twice and did not allow LPA to inspect. Based on today's inspection, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care. Inspection authority regulation was explained to the licensee, Helen Chen. If this occurred again, civil penalty of $500 will be issued.

Resolution: The licensee arrived at the facility 30 mins later and LPA and the licensee entered the facility together. The deficiency has been corrected.
