Based on observation, interview, record review, the licensee did not comply with the section cited above in which staff #1 not having a current mandated reporter certification, poses/posed a potential health, safety or personal rights risk to persons in care. Director stated that staff #1 was on leave and returned on _.
Based on observation, interview, record review, the licensee did not comply with the section cited above by the facility not having any fire extinguishers on site that have been timely serviced within the last year for the fire clearance which poses a potential health, safety or personal rights risk to persons in care. The fire extinguishers in the classrooms were serviced on 11/21/23, however director produced invoice which states they were last serviced on 5/30/24.
