ROSALES FAMILY CHILD CARE

Based on observation, interview, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care. LPA Garcia observed unsafe conditions in bathroom utilized by the children due to remodel/ construction; such as exposed wood beams with large splinters, exposed electrical equipment, tools. LPA Also observed the backyard patio not have a staircase for the children to safely access the outdoor area. Licensee stated and LPA observed, she placed a stepping stool to access the yard.

Resolution: Licensee stated that the bathroom would be completed by this weekend and will have workers make a staircase with hand rails to safely access the backyard. Children will not utilize backyard until the staircase and handrail are complete.

Based on observation, interview, record review the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care. Licensee received a technical violation in 2024 for not completing Preventative Health and Safety certification, and licensee states she has not completed it.

Resolution: Licensee will call CCRC to see when next available Preventative Health and Safety will be conducted so she can enroll. Licensee will send proof to LPA when she is enrolled and when the certification is complete.

Based on record review, the licensee did not comply with the section cited above. Licensee was unable to find current immunizations for children #1-3, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee shall document and keep immuization records for each child enrolled. Licensee will send a copy of children #1-3 immunzations to LPA Tamayo no later than 08/31/23.

Based on record review, the licensee did not comply with the section cited above. Licensee was unable to find current emergency information LIC700 for each child enrolled, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee shall ensure she has copies of LIC700 for each child enrolled. Licensee will send copies of LIC700 to LPA Tamayo no later than 08/31/2023.
