Based on children's record review, the licensee did not comply with the section cited above in one (1) out of five (5) children's which poses a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee will submit proof of the updated blue card to the LPA by 08/18/2025.
Based on LPA observation and licensee interview, the licensee did not comply with the section cited above in which a Baby Einstein Neighborhood Symphony Activity Jumper in the side yard of the home, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: The Licensee stated it was left behind over the weekend and will be removed by 07/19/2025.
Based on LPA observation and licensee interview, the licensee did not comply with the section cited above at 9:23 AM LPA observed three (3) blankets inside the play yard. Licensee admitted the infant sleeps with two (2) blankets, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee removed the blankets from the play yard during the inspection.
Based on LPA observation, the licensee did not comply with the section cited above in which the LPA observed a open area located between the Play Room and the Dining Room with exposed cables. Outdoors six (6) 2 x 4 wood against the left side yard, wood paneling around the deck that is in disrepair behind the children's outdoor play set, accessible staircase leading to the deck, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee's husband remived the six (6) 2x4 wood and re-installed the door leading to the deck. The wood paneling behind the play set was also repaired during the inspection. The licensee will submit proof once the staircase is made inaccessible to children in care as well as the exposed cables inside the home by 08/18/2025.
Based on LPA observation and record review, the licensee did not comply with the section cited above by not renewing her expired Mandated Reporter Training which expired on 03/14/2025, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee will submit proof upon completion via email to the LPA by 08/18/2025.
Based on LPA observation and licensee interview, the licensee did not comply with the section cited above. At 9:00 AM upon arrival LPA observed door to off-limit area described on LIC 999 Sketch as storage accessible to children in care. Additionally, the licensee admitted to putting the infant down for nap time in an off-limit area which poses a potential health, safety or personal rights risk to persons in care.
Resolution: During the inspection the licensee locked the off-limit area and removed the play yard from the off limit area. The licensee stated she will submit a written declaration that she understands that off-limit areas need to be inaccessible at all times during daycare hours of operation.
Based on licensee and(P1) for (C1) interview and record review, the licensee did not comply with the section cited above in two (2) out of five (5) children's records reviewed which poses a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee stated she will provide proof of the updated immunizations to the LPA by 08/18/2025.
Based on interview and record review, the licensee did not comply with the section cited above in not having the AB-1207 Mandated Reporter certificate which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Provider stated that she will ensure to take the required training and send a copy of the certificate to the San Diego Child Care Regional Office (SDCCRO) as proof of correction.
