Based on observation, Co-Licensee's statement, and record review the licensee did not comply with the section cited above in that neither co-licensee or two (2) assistants have completed the Mandated Reporter Training course, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: LPA provided Licensee with written instructions on how to locate the Mandated Reporter AB1207 training online. Licensee stated co-licensees and assistants will complete this online course and provide LPA with a copy of the training completion certificates
Based on interview and record review, co-licensee did not have the required immunization's (MMR, TB, Dtap, Influenza) on file for co-licensee's or two (2)assistans wich poses a potential health, safety or personal rights risk to persons in care.
Resolution: Co-licensee stated she will ensure that co-licensees and two (2) assitnats receive the necessary immunizations and will send a copy to the CCLD office by 8/26/22.
Based on observation, interview and record review, the licensee did not comply with the section cited above in that ten (10) out of (10) children's files did not include the LIC282 Liability Insurance form which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Co-licnesee stated she will ensure parents receive and turn in the completed LIC282 Liability Insurance form by 8/26/22. Licensee will send copies of completed forms to the CCLD office by 8/26/22.
