Based on record review, the licensee did not comply with the section cited above in not providing the Department with a current mandated reporter training certificate, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will send the department a copy of a current mandated reporter training certificate by email by 10/31/25. Licensee stated she will add this reminder to her calendar to ensure this citation is not repeated.
Based on record review, the licensee did not comply with the section cited above as she has not been maintaining safe sleep logs for the one infant she has enrolled which poses/posed a potential health, safety or personal rights risk to children in care.
Resolution: Licensee stated that she was not aware of the safe sleep record requirements for infants. Analyst provided licensee with a copy of the updated infant regulations and a sample safe sleep log during today's visit. Licensee states she will keep track of infant's sleep from this point forward beginning tomorrow and send analyst a copy of the log kept for the period of 05/16/23 to 05/19/23 and send to analyst by 05/22/23 to complete the correction.
Based on analyst record review, the licensee did not comply with the section cited above as she does not have a current Mandated Reporter Training certification which poses/posed a potential health, safety or personal rights risk to children in care.
Resolution: The licensee stated she will complete a Mandated Reporter Training Class at www.mandatedreporterca.org and send analyst a copy of the certificate of completion by 06/12/23 to complete the correction.
