THIS IS AN AMENDED REPORT - 101229 Responsibility for Providing Care and Supervision (a) The licensee shall provide care and supervision as necessary to meet the child’s needs. (1) No child shall be left without the supervision of a teacher at any time…Supervision shall include visual observation. This requirements was not met as evidenced by… Based upon staff interviews and facility’s unusual incident report, the licensee did not ensure child (C1) was supervised and was left alone for several minutes which is an immediate health, safety and personal rights risk to children in care.
(2) Each person specified in (g) above shall have a health-screening report signed by the person performing the screening. This report shall indicate the following:
