Based on interview and record review, the licensee did not comply with the section cited above as she stated she has not conducted fire and disaster drills in the past six months which poses/posed a potential health, safety or personal rights risk to children in care.
Resolution: Licensee states she will conduct both drills with the children in care and document them on a drill log which she will send to analyst by 03/10/25 to complete the correction.
Based on interview and record review, the licensee did not comply with the section cited above as she was not maintaining records for her napping infants which poses/posed a potential health, safety or personal rights risk to children in care.
Resolution: Licensee was not aware that napping records must be kept for infants from 12-24 months and was not keeping them for the older infants she currently has enrolled. Analyst provided licensee with a copy of a sample safe sleep log. Licensee states she will begin observing and documenting all her infants napping starting tomorrow, 02/25/25, for all her three infants in care and submit copies of the records she keeps for one of the children of her choice, for the period of 02/25/25-03/07/25, to analyst by 03/10/25 to complete the correction.
Based on interview and record review, the licensee did not comply with the section cited above as both she and spouse, who assists with the day care, have not completed the required mandated reporter training which poses/posed a potential health, safety or personal rights risk to children in care.
Resolution: Analyst provided licensee with the mandated reporter training website information and licensee and spouse state they will take the training and submit a copy of the certificate of completion to analyst by 03/24/25 to complete the correction.
Based on interview and record review, the licensee did not comply with the section cited above as she stated she has not conducted fire and disaster drills in the past six months which poses/posed a potential health, safety or personal rights risk to children in care.
Resolution: Licensee states she will conduct both drills with the children in care and document them on a drill log by 11/18/22. Analyst will conduct a return plan of correction visit to ensure that the correction has been completed.
Based on analyst observation, the licensee did not comply with the section cited above as an infant in care was sitting in a disallowed rocking device when analyst arrived which poses/posed a potential health, safety or personal rights risk to child in care.
Resolution: The deficiency was cleared when child was moved to a baby swing and licensee spouse moved the disallowed equipment from the for use day care area. Licensee was provided with a prohibited day care equipment flyer and understands which pieces of infant equipment are not allowed in day care.
Based on interview and record review, the licensee did not comply with the section cited above as she was unaware of the safe sleep regulations and was not maintaining records for napping infants which poses/posed a potential health, safety or personal rights risk to children in care.
Resolution: Analyst provided licensee with a copy of a sample safe sleep log. Licensee states she will begin observing and documenting all her infants napping starting tomorrow, 11/09/22. Analyst will conduct a return plan of correction visit to ensure that the correction has been completed.

Based on interview and record review, the licensee did not comply with the section cited above as her spouse, who assists with the day care, did not have a copy of his immunizaton records on file at the facility which poses/posed a potential health, safety or personal rights risk to children in care.
Resolution: Licensee states spouse will obtain a copy of his vaccination records or proof of immunity from his physician and provide those records to analyst by 12/12/22 to complete the correction.
Based on record review, the licensee did not comply with the section cited above as child #3 did not have any records on file which poses/posed a potential health, safety or personal rights risk to children in care.
Resolution: Licensee was provided a blank records packet for the parent of the child in question to complete and will have her fill it out and return it to complete the correction. Analyst will conduct a return plan of correction visit to ensure that the correction has been completed.
Based on record review, the licensee did not comply with the section cited above as her three infants in care did not have completed infant sleeping plans which poses/posed a potential health, safety or personal rights risk to children in care.
Resolution: Licensee was provided with a copy of the infant sleeping plan form and states will have parents complete them and place them in their respective files by 11/18/22. Analyst will conduct a return plan of correction visit to ensure that the correction has been completed.
