VILLALPANDO, ALEXIS & ELIZABETH FAMILY CHILD CARE

Based on record review, the licensee did not comply with the section cited above. Mrs. Elizabeth Villalpando did not complete a course on Preventative Health Practices which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Alexis Villalpando stated that she will have Mrs. Elizabeth Villalpando register for a class no later than 11/08/2023. A copy of the registration will be email'd no later than 11/08/2023.

Based on observation and interview, the licensee did not comply with the section cited above. LIC 610 was not posted and no proof was availalbe that disaster drills have been conducted at least every 6 months which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensees will post LIC 610A, conduct fire drill and document on log, and submit photo proof to LPA by emiail by POC due date of 3/3/22.

Based on interview, the licensee did not comply with the section cited above as she stated records were not availalbe on site for facility staff or children which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit copies of all personnel records, including current CPR/FA, Mandated Reporter certification, LIC 9052, and other records required for personnel to LPA by POC due date of 3/3/22.

Based on interview with licensee, Alexis, the licensee did not comply with the section cited above, no records were availalbe to review at the child care for any staff or children which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide copies of completed child files for all children enrolled including a facility roster and documentation of immunizations along with declaration explaining reason records were not available and plan to maintain compliance by POC due date.
