Based on interview and record review, the licensee did not comply with the section cited above in one out of one record reviews which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee will provide proof of conducting a fire and disaster drill and documenting to LPA via email by 04/25/2025.

Based on record review, the licensee did not comply with the section cited above in one out of six children's records reviewed by LPA, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee will provide the LPA proof of the completed LIC 9227 Individual Infant Sleeping Plan by 04/25/2025 and will submit a declaration the regulation was discussed and she understands.

Based on record review, the licensee did not comply with the section cited above in one out of one staff files review which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee will provide the LPA with proof of TDAP, MMR, TB and Influenza vaccine or statement declining the influenza vaccine. The licensee will email the LPA the proof by 04/25/2025.

Based on record review, the licensee did not comply with the section cited above in one out of one staff files which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee will provide the LPA with proof upon completion of the mandated reporter training by 04/25/2025 via email.

Based on observation and interview, the licensee did not comply with the section cited above in two out of three backyard fence areas which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee will provide the LPA with proof of left and right side of the backyard fence once it has been repaired. The left side where the children's outdoor play toys are kept is loose. The right side next to the playyard structure is being held up by belts. The licensee will provide the LPA proof of repairs via email by 04/25/2025.

Based on record review, the licensee did not comply with the section cited above in one out of six children's records reviewed by LPA, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee will provide the LPA proof of the emergency information card for child 1 via email by 04/25/2025.

Based on observation and interview, the licensee did not comply with the section cited above in 5 out of 5 children, as knives in a kitchen drawer were accessible to children and all off limit areas were accessible to children which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee removed the knives from the drawer during the inspection, licensee will purchase door knob covers for all off limit areas and provide a photo to LPA Willliamson, no later than 2/15/23.

Based on interview and record review, the licensee did not comply with the section cited above in 5 out of 5 children, as licensee did not have immunization records available for review during time of inspection which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will complete Mandated Reporter training and provide LPA Williamson with a copy of training certificate, no later that 2/28/23.

Based on interview and record review, the licensee did not comply with the section cited above in 5 out of 5 children, as there was not a Children's Roster available for review during time of inspection which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will complete Children's Roster and provide a copy to LPA Williamson, no later than 2/10/23

Based on observation and interview, the licensee did not comply with the section cited above in 5 out of 5 children, as the smoke detector was not operational during time of inspection which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee has agreed to purchase a smoke detector and provide LPA with a photo and video of the smoke detector, no later than 2/9/23.
