Based on observation, interview, and record review, the licensee did not comply with the section cited above. One adult resident did not obtain criminal record clearance or exemption, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will complete Fingerprints for adult son, Erick Reynoso, and turn in as proof a completed LIC9163 BY THE NEXT DAY 01/30/2026.

Based on observation, interview, and record review, the licensee did not comply with the section cited above. Licensee stated that Emergency Drills have not been performed or documented as of June of 2026, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that an Emergency Drill will be performed soon and documented. Proof will be sent to LPA Macias by 2/06/2026.

Based on observation, interview, record review, the licensee did not comply with the section cited above. Licensee stated that sleep logs have not been performed which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that going forward, Sleep Logs will be updated daily, documenting infant's 15 min check up while they sleep.

Based on observation, interview and record review, the licensee did not comply with the section cited above. Licensee and helper did not renew the Mandated Reporter Certification, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that her helper and herself, will take take the Mandated Reporter Training and send certification to LPA Macias by 2/06/2026.

Based on observation, interview, and record review, the licensee did not comply with the section cited above. Children Records are missing for 1 out of 11 children enrilled, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will obtained a signed Children's Records Packet for the new child enrolled, and provide proof to LPA Macias by 2/06/2026.

Based on observation, interview and record review, the licensee did not comply with the section cited above. Licensee did not have an updated Roster to provide to licensinng duruing inspection, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: A complete and updated roster will be turned in to LPA Macias by 2/06/2026.

Based on observation, interview, and record review , the licensee did not comply with the section cited above. Licensee was providing care and supervision to 11 children in care without a fingerprint cleared and associated helper, which posed a potential health safety or personal rights risk to persons in care.

Resolution: Licensee stated she would provide proof of completed LIC 9163 Form by 1/30/2026.

Based on observation, the licensee did not comply with the section cited above in that there were two pack and plays occupied with awake children, 12 and 14 months, that had blankets and a boppy pillow inside. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee removed the items today and will follow the safe sleep regulations.

Based on record review, the licensee did not comply with the section cited above in infants #1 and #2, a 5 and 10 month old, did not have this form on file which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states that she will provide the parents the forms when they pick up their children today and will provide copies to Licensing to verify correction by 3/17/23.

Based on record review, the Licensee did not comply with the section cited above in that there were no sleep logs for the children under two years, which poses a potential health, safety or personal rights risk to persons in care.

Based on observation and record review, the licensee did not comply with the section cited above in that there were five children present under the age of two, Children #1 to 5, 5 mo., 10 mo. 1 year, 14 mo. and 23 mo. which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states that she though Child #5 had already turned two. Children #3 and 4 only come a couple times a week and she will ensure that there will be nore more than 4 children under the age of two here at any time. Licensee will provide a schedule for the five children to verify correction and send to Licensing by 3/17/23.
