Resolution: Licensee will provide parents with required licensing forms to complete. LPA reviewed LIC311D resource with licensee which lists required documents for each child's file. Licensee will submit copies of children's completed files to the San Diego Regional office by 3/21/2025.

Based on observation the licensee did not comply with the section cited above in that cleaning products, a bucket of water, and knives were accessible to children in the kitchen lower cabinet, floor, and drawer which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will make the items inaccessible to children through locks or a safety gate. Licensee will submit photos of the drawers and cabinets showing they are now inaccessible to children. Licensee will submit photos of correction to the San Diego Regional Office by 2/24/2025.

Resolution: Licensee stated she will purchase a crib or play yard for infant in care and submit the receipt and photo of item to the San Diego Regional office by 2/24/2025.

Based on observation and record review, the licensee did not comply with the section cited above in that assistant (A1) did not have a criminal record clearance, was interacting and watching children and stated she helps out occasionally which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will ensure assistant does not return to work until a fingerprint clearance is obtained. Licensee will submit the proof of clearance to the San Diego Regional Office.

Based on observation, record review, and interview the licensee did not comply with the section cited above in that the two (2) assistants present during the inspection and alone with children, did not have a valid CPR and First Aid card available for review which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will ensure at least one (1) person present, caring for the children, possess a valid and current CPR card. Licensee will submit proof of valid CPR and First Aid cards to the San Diego Regional office by 3/3/25.

Based on record review the licensee did not comply with the section cited above in that the last fire drill was conducted and documented on 9/24/24 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will conduct and document a fire drill by 3/7/24 and schedule other safety drills to be conducted every six( 6) months. Licensee will submit the documentation to the San Diego Regional office by 3/7/25.

Resolution: Licensee stated she will ensure infants sleep is documented every 15 minutes. LPA provided samples of sleep logs that can be used. Licensee will submit documentation to the San Diego Regional office by 2/28/25.
