Based on record review, the licensee did not comply with the section cited above in one out of two staff which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee will send proof of enrollment and send certificate upon completion via email or text by 05/29/2025.

Based on record review, the licensee did not comply with the section cited above in four out of six children's files which poses a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above in three out of six children's files which poses a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above in one out of one roster which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee will provide the LPA with a current roster and send via email or text by 05/29/2025.

Based on record review, the licensee did not comply with the section cited above in two out of three staff which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee will send proof upon completion via email or text by 05/29/2025.

Based on analyst observation and interview, the licensee did not comply with the section cited above as she has not been conducting fire and disaster drills every six months as required which poses/posed a potential health, safety or personal rights risk to children in care.

Resolution: Licensee was provided with a blank fire/earthquake drill log and states will conduct both drills with the children in care this week and then continue to do so once every six months as required by regulation. Licensee states she will submit a copy of the completed drill log to analyst by 03/31/23 to complete the correction.

Based on analyst interview and record review, the licensee did not comply with the section cited above as she is not documenting the infant she has in care's sleeping status every 15 minutes as required by regulation which poses/posed a potential health, safety or personal rights risk to the child in care.

Resolution: Licensee was provided with a sample Safe Sleep log and stated she will begin documenting infants sleeping status every 15 minutes. The Licensee will send a copy of the completed log for the dates of 3/27/23 - 3/31/23 to analyst by 3/31/23.

Based on observation and Licensee's statement that she has not taken the Mandated Reporter Training, the licensee did not comply with the section cited above in that she has not completed the Mandated Reporter Training course, which poses as a potential health, safety or personal rights risk to persons in care.

Resolution: LPA provided Licensee with written instructions on how to locate the Mandated Reporter Training online. Licensee states she will complete this online course and provide LPA with a copy of her training completion certificate no later than 04/26/2023.

Based on analyst record review, the licensee did not comply with the section cited above as she did not have any record files for children in care which poses/posed a potential health, safety or personal rights risk to children in care.

Resolution: Analyst provided licensee with a blank records packet to provide to the parent of the one family of children she has in care to complete and submit copies of to analyst by 04/07/23 to demonstrate appropriate record keeping practices for children in care.

Based on observation and Licensee's statement that there is no LIC 9227 form for the infant, the licensee did not comply with the section cited above in that one (1) out of one (1) infants lacks a completed LIC 9227 Individual Sleeping Plan on file, which poses as a potential health, safety or personal rights risk to children in care.

Resolution: LPA provided Licensee with a blank LIC 9227 form and PIN 20-24-CCP "Safe Sleep Regulations". The Co-Licensee stated she will submit a copy of the completed LIC 9227 for the infants in care for the plan of correction to the San Diego Regional Office by 331/2023.

Based on analyst record review, the licensee did not comply with the section cited above as it was determined that she is not maintaining a facility roster which poses/posed a potential health, safety or personal rights risk to children in care.

Resolution: The licensee provided the roster while LPA was still present during inspection.
