Based on analyst record review, the licensee did not comply with the section cited above as licensee is not maintaining safe sleep records for the only infant che has in care, (child #1), which poses/posed a potential health, safety or personal rights risk to the infant in care.
Resolution: Analyst provided licensee with a blank copy of a safe sleep log and licensee states she will document the infant in care's naps beginning tomorrow, 02/11/26, through 02/20/26 and continue to monitor and documenting his naps going forward and then send analyst a copy of the log through the 02/11/26-02/20/26 period by 02/23/26 to correct the deficiency.
Based on analyst record review, the licensee did not comply with the section cited above as she did not have copies of required immunizations for assistant #1 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states she will have the assistant locate her copies of the required vaccinations from her medical provider or, if she cannot, provide proof of immunization or obtain the required shots and send analyst a copy of the documenation by 03/09/26 to correct the deficiency.
Based on analyst observation, the licensee did not comply with the section cited above as during today's visit the kitchen cabinet beneath the sink was unlatched and contained various cleansers while a unlatched kitchen drawer contained knives which poses/posed a potential health, safety or personal rights risk to children in care.
Resolution: The violation was cited as a type B violation as the cleansers were all properly sealed and the children had a strong two provider to four children supervision ratio today. The licensee removed all the hazardous items today and moved them to an inaccessible area, however, states she will install latches in the cabinet and drawer and send analyst a photo of their installation by 11/04/24 to complete the correction.
Based on analyst observation, the licensee did not comply with the section cited above as the playpen for child #1 had various toys inside and attached to the side of it which poses/posed a potential health, safety or personal rights risk to children in care.
Resolution: The deficiency was corrected today as licensee removed all the attached toys leaving the playpen clear of any items. Licensee understands that an infant's sleeping equipment (play yard, crib, bassinet, etc.) must always be free of any items in or around it.
Based on analyst interview and facility record review, the licensee did not comply with the section cited above as she has not been maintaining a safe sleep log for the infants she has in care which poses/posed a potential health, safety or personal rights risk to children in care.
Resolution: Licensee states she will begin maintaining a safe sleep log starting tomorrow and continuing forward and then will submit a copy of the sleep logs for both infants for the period of 10/18/22-10/30/22 to analyst on 10/31/22 to complete the correction.
Based on analyst interview and facility record review, the licensee did not comply with the section cited above as she did not obtain an LIC 9227 for the five month old infant she has in care which poses/posed a potential health, safety or personal rights risk to children in care.
Resolution: Analyst provided licensee with a copy of the a blank LIC 9227 Individual Infant Sleeping Plan for licensee to give to infant's parent to complete. Licensee states she will maintain the form in the child's file and then submit a copy of it to analyst by 10/24/22 to complete the correction.
