TINEUS, ROSE MARIE FAMILY CHILD CARE

Based on analyst interview and record review, the licensee did not comply with the section cited above as the licensee has not been conducting fire and earthquake drills as required which poses/posed a potential health, safety or personal rights risk to children in care.

Resolution: When asked whether she had been conducting disaster drills, licensee stated that she hadn't. Analyst provided her with a blank disaster drill log and licensee stated she will conduct both drills, document them and send them to analyst by 02/16/26 to correct the deficiency and then continue to document them going forward.

Based on analyst record review, the licensee did not comply with the section cited above as she did not have emergency medical forms for any of the enrolled children which poses/posed a potential health, safety or personal rights risk to children in care.

Resolution: Licensee also did not have notification of parent's rights acknowledgement, additional children in care form or shot records transferred to blue cards. Analyst provided copy of each form for licensee to provide to respective parents to complete and licensee will submit copy of missing records for three children of her choice to analyst by 02/16/26 to correct the deficiency.

Based on analyst record review, the licensee did not comply with the section cited above as her roster was missing the names of children currently enrolled in the day care which poses/posed a potential health, safety or personal rights risk to children in care.

Resolution: Analyst provided licensee with a blank roster form to redo or add to her current roster and she states, once completed, will send copy to analyst by 02/16/26 to correct the deficiency.
