Based on record review, the licensee did not comply with the section cited above in Licenee did not provide a current mandated reporter cerficate for Staff #1, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will send the department poof of mandated reporter certificate for Staff #1 by 5/15/25 by email.

Based on record review, the licensee did not comply with the section cited above in not providing the department with immunization records for Child #1 and Child #2, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will submit to the department a copy of a current immunization record for Child #1 and Child #2 by eamil by 5/15/25.

Based on analyst interview and record review, the licensee did not comply with the section cited above as she is not documenting the infants she has in care's sleeping status every 15 minutes as required by regulation which poses/posed a potential health, safety or personal rights risk to the child in care.

Resolution: Licensee was provided with a sample Safe Sleep log and stated she will begin documenting infants sleeping status every 15 minutes. The Licensee will send a copy of the completed log for the dates of 2/17/23 - 2/24/23 to analyst by 2/24/23.

Based on observation and Licensee's statement that she or assistant (S1) have not taken the Mandated Reporter Training, the licensee did not comply with the section cited above in that they have not completed the Mandated Reporter Training course, which poses as a potential health, safety or personal rights risk to persons in care.

Resolution: LPA provided Licensee with written instructions on how to locate the Mandated Reporter Training online. Licensee states she and (S1) will complete this online course and provide LPA with a copy of their training completion certificate no later than 3/17/2023.

Based on interview and record review, the licensee did not comply with the section cited above in that (1) out of (1) asssitant (S1) personnel records were available to review which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: LPA provided Licensee with LIC311D form which lists forms needed for employees. Licensee states she will complete this packet for (S1) and provide LPA with a copy of the required forms no later than 03/03/2023.

Based on interview and record review, the licensee did not comply with the section cited above in (3) out of (3) persons did not have immunizations available for review which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states she will submit written documentation of required TB, MMR, Dtap, Influenza immunizations for licensee, adult resident and (S1) no later than 03/03/2023.

Based on record review and Licensee's statement that she or assistant's (S1) Preventive Health class was not available for review, which poses as a potential health, safety or personal rights risk to persons in care.

Resolution: LPA provided Licensee with written instructions on how to locate the Preventive Health training online. Licensee states she and (S1) will complete this online course and provide LPA with a copy of her training completion certificate no later than 03/17/2023.

Based on analyst observation and interview, the licensee did not comply with the section cited above as she has not been conducting fire and disaster drills every six months as required which poses/posed a potential health, safety or personal rights risk to children in care.

Resolution: Licensee was provided with a blank fire/earthquake drill log and states will conduct both drills with the children in care by next week and then continue to do so once every six months as required by regulation. Licensee states she will submit a copy of the completed drill log to analyst by 02/24/23 to complete the correction.
