Based on interview and record review, the licensee did not comply with the section cited above. Mrs. Ta did not maitain a sleep plan for one infant in care. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Mrs. Ta stated that she will provide the sleeping plan form to parent and have them complete the form and submit a copy to the department no later than 7/9/2024.

Based on observation, the licensee did not comply with the section cited above. LPAs observed several items accessible to children in the back patio area: Stationary bike, electric saw, boxes of canned goods and other miscellaneous items. This poses an immediate health, safety or personal rights risk to persons in care.

Based on interview and record review, the licensee did not comply with the section cited above. Mrs. Ta has not conducted a fire drill since July 2023. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Mrs. Ta stated that she will conduct a fire drill and submit a copy of the fire drill log to the department no later than 7/9/2024. She understands that a fire drill/emergency drill must be conducted twice a year (once every 6 months).

Based on interview and record review, the licensee did not comply with the section cited above. Mrs. Ta did not maintain the 15-minute sleep checks for the two infants in care. This which poses a potential health, safety or personal rights risk to persons in care.

Resolution: LPA reviewed the sleep regulation with Mrs. Ta. She stated that she will maintain the 15-minute nap checks for the two infants in care and submit copies of the log beginning today through 7/9/2024.

Based on observationi, the licensee did not comply with the section cited. LPA observed a baby walker and a baby rocker present in the child care area. Mrs. Ta removed the 2 prohibited items immediately. She was reminded that these items are prohibited. She stated that she understands the regulation. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Corrected today 12/1/2022.

Based on record review, the licensee did not comply with the section cited above. Licensee's spouse, Meng Doan is a helper. Mrs. Ta was unable to locate a copy of his immunization record. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Mrs. Ta shall submit a copy of Meng Doan's immunization no later than 12/08/2022 to show that he was immunized against Pertussis, Measles and Influenza.

Based on record review, the licensee did not comply with the section cited above. Mrs. Ta did not maintain a sleeping plan for a 10-month old child in care. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Form LIC 9227 was provided today. Mrs. Ta shall provide form to parent of child for completion and submit a copy to the department no later than 12/08/2022.

Based on record review, the licensee did not comply with the section cited above. Mrs. Ta failed to maintain the required 15-minute check log for 3 infants in care. This poses/posed a potential health, safety or personal rights risk to persons in care. A sample copy of nap log was provided to Mrs. Ta today.

Resolution: Mrs. Ta shall maintain a 15-minute check nap log for all infants in care. She will submit a one-week's worth (copies) of nap log to the department no later than 12/08/22.
