Based on analyst observation, the licensee did not comply with the section cited above as she did not have a fire extinguisher or smoke detector available and operating which poses/posed a potential health, safety or personal rights risk to children in care.

Resolution: Licensee states she will purchase a new fire extinguisher and smoke detector and send analyst a photo of the purchase receipt, video of the smoke detector installed and being tested, and fire extingusiher gauge by 03/07/23 to complete the correction.

Based on record review, the Licensee did not comply with the section cited above in that Licensee’s Mandated Reporter Training has expired which poses a potential health, safety or personal rights risk to persons in care.

Resolution: LPA provided Licensee with written instructions on how to locate the Mandated Reporter Training online. Licensee states she will complete this online course and provide LPA with a copy of her training completion certificate no later than 03/21/2023.

Based on observation, interview and record review, the Licensee did not comply with the section cited above in that she did not have immunization records for TB, Dtap, MMR, and Influenza on file for review which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will submit proof of immunization's by 3/17/23 to CCLD.

Based on interview and record review, the Licensee did not comply with the section cited above as the Licensee’s CPR/First Aid certification has expired which poses a potential health, safety or personal rights risk to children in care.

Resolution: Licensee stated she will enroll in an EMSA CPR/First Aid training class, complete it and will submit a copy for the plan of correction to CCLD by 3/21/2023. Licensee will ensure to renew her certification every two years prior to the expiration date.

Based on interview and record review, the licensee did not comply with the section cited above in that licensee did not report to CCLD the fire in the home April 2022 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will review training video on the website: https://ccld.childcarevideos.org/family-child-care-providers/ called Child Care Reporting Requirements. Licensee will submit a summary to CCLD after viewing video by 3/3/2023.
