Type A citation - immediate risk to health, safety, or personal rights

Based on LPA's interviews with staff members and a witness the facility staff failed to maintain constant supervision and visual observation of a daycare child. This poses an immediate health and safety risk to children in care.

Based on interview, and record review, the Licensee did not comply with the section cited above in that an individual was caring for children, without being criminally cleared and associated to the facility, which poses an immediate health, safety or personal rights risk to persons in care.

§1596.8662 (b) (1)   Availability of information regarding detecting and reporting child abuse and neglect; training for mandated reporter who is licensed day care provider, administrator, or employee of a licensed child day care facility; proof of completion (b) (1)   On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training.
