Based on observation, interview, and record review, the licensee did not comply with the section cited above in that the licensee has not completed the Mandated Reporter training and assistant S1's training expired in 2018 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: LPA provided Licensee with written instructions on how to locate the Mandated Reporter Training online. Licensee stated she and assistant S1 will complete this online course, and provide LPA with a copy of their training completion certificate no later than 08/16/2023.
