Based on analyst record review, the licensees did not comply with the section cited above as the facility did not have a roster to review during today's visit which poses/posed a potential health, safety or personal rights risk to children in care.
Resolution: Licensee Said stated that he maintains the facility roster digitally but has to update it to only include current day care children. Licensee stated he will update his roster and send analyst a copy by 01/06/25 to commplete the correction. Licensees understand a current roster must always be maintained at the facility for analysts to review during any in person visit and at any given time.
Based on analyst interview and record review, the licensees did not comply with the section cited above as they stated they have not been keeping infant sleep logs for the infants they have in care which poses/posed a potential health, safety or personal rights risk to infants in care.
Resolution: Licensees were provided with a sample safe sleep log sheet and stated that they will complete its entries every time infants nap in the home going forward. Licensees state they will begin maintaining the log when chiildren return to care on 01/06/25 and send him a copy of the entries from that day through 01/10/25 by 01/13/25 to complete the correction.
Based on analyst record review, co-licensee Said's Mandated Reporter Training Certificate did not comply with the section cited above as it expired on 12/10/24 which poses/posed a potential health, safety or personal rights risk to children in care. Co-licensee Aden is exempt from the requirement.
Resolution: Licensee Said states he will complete the on-line training at mandatedreporterca.com and send analyst a copy of the certificate of completion by 01/27/25 to correct the deficiency.
Based on analyst record review, the licensees both did not comply with the section cited above as they did not have current copies of required CPR/First Aid certifications which poses/posed a potential health, safety or personal rights risk to children in care.
Resolution: Licensees state they will attempt to locate copies of their most recent trainings and if unable to find them will enroll in an EMSA approved training course and send analyst a completed training certificate by 01/27/25 to correct the deficiency.
Based on analyst record review, the licensees did not comply with the section cited above as they did not have any records on file for any children in care which poses/posed a potential health, safety or personal rights risk to children in care.
Resolution: Licensee Said stated he maintains the records digitally for children in care and will submit a copy of them for four children of his choosing and update all other files and then send analyst a copy of those four files by 01/13/25 to complete the correction. Licensee also understands that children's files must always be available for review by department analysts and should maintain accessible copies at the facility going forward.
Based on analyst initial record review, the licensees did not comply with the section cited above as the facility's disaster plan was not posted in the day care which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: The deficiency was corrected as, after the citation was issued, the disaster plan was located in a notebook that contained other licensing paperwork. Licensees were advised to place the plan in the notification posting area in their home to maintain compliance.
Based on analyst observation and record review, the licensees did not comply with the section cited above as their last logged time conducting fire and earthquake drills was 12/10/22 which poses/posed a potential health, safety or personal rights risk to children in care.
Resolution: Licensee Said stated that they have conducted drills since that time but have not logged them. Licensee states he will document all drills conducted since that time and conduct a current drill for the most recent six months and add that to the log if it has not been completed yet and then submit of copy of the updated log to analyst by 01/13/25 to complete the correction.
Based on analyst record review, the licensees did not comply with the section cited above in as they have not been keeping safe sleep logs for the one infant they have in care, child #1, which poses/posed a potential health, safety or personal rights risk to children in care.
Resolution: Licensees stated they were just recently made aware of the safe sleep log and have not been keep a record as required. Analyst provided licensees with a copy of a blank log and licensees state they will keep a log for every infant they enroll going forward. To correct the deficiency, licensees will submit the log to analyst by 07/31/12 for the days from 07/25/23-07/28/23.
Based on record review, the licensees did not comply with the section cited above as they did not have any children's files immediately available for analyst to review during his inspection visit for the visiting family of children, or any enrolled children, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensees state that they have digital copies of files and will complete records for the six children who attend the facility on an occasional basis and also submit a sample of three records of their choosing for enrolled children to analyst by 07/31/23 to complete the correction.
Based on record review, the licensees did not comply with the section cited above as they did not have a faciity roster which poses/posed a potential health, safety or personal rights risk to children in care.
Resolution: Licensees stated they have a digital copy of the roster and will e-mail it to analyst by 07/31/23 to complete the correction. Licensees understand that a roster must be available in some format at all times to have available for an analyst to review during an inspection visit.
Based on analyst observation, the licensees did not comply with the section cited above as the play yard in the facility did not contain a fitted sheet but, instead, had a blanket that was tied to the underside of the matress which poses/posed a potential health, safety or personal rights risk to children in care.
Resolution: Licensees state they will by a fitted sheet for the play yard and replace the blanket with it. Licensees will then take a picture of the sheet on the mattress and send it to analyst with a copy of the purchase receipt by 07/31/23 to complete the correction.
