Based on analyst record review, the facility did not comply with the section cited above as staff members #3 and #5 did not have current mandated reporter certifications which poses/posed a potential health, safety or personal rights risk to children in care.

Based on interviews and record reviews, a lack of supervision occurred for Child #1 (C1) during a 10/30/24 self-reported incident at the facility when she left her classroom unnoticed and was gone for a reported twelve minutes before staff was made aware of her absence which posed an immediate Health and Safety risk to the child in care.

Based on analyst record review, the facility did not comply with the section cited above as staff member #2 did not have documented proof of a required MMR vaccination which poses/posed a potential health, safety or personal rights risk to children in care.

Based on analyst record review, the licensee did not comply with the section cited above as none of the record reviews demonstrated that any of the pm classroom children had obtained a written medical assessment which poses/posed a potential health, safety or personal rights risk to children in care.
