Based on observation and interview, the licensee did not comply with the section cited above in that the back fence was unstable and positioned where hazardous poisons, tools, and electrical cords were accessible to children. The side yard was accessible to children which contains dog feces which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: The licensee state she will replace the unstable fence and add a side yard fence to make the hazardous items inaccessible to children. Licensee also stated she will ensure she implements a checklist and check review to ensure the areas are safe for children.
Based on analyst interview and record review, the licensee did not comply with the section cited above as she is not documenting the infant she has in care's sleeping status every 15 minutes as required by regulation which poses/posed a potential health, safety or personal rights risk to the child in care.
Resolution: Licensee was provided with a sample Safe Sleep log and stated she will begin documenting infants sleeping status every 15 minutes. The Licensee will send a copy of the completed log for the dates of 5/14/24 - 5/21/24 to analyst by 5/21/24.
Based on analyst observation and interview, the licensee did not comply with the section cited above as she has not been documenting fire and disaster drills every six months as required which poses/posed a potential health, safety or personal rights risk to children in care.
Resolution: Licensee was provided with a blank fire/earthquake drill log and states will conduct both drills with the children in care this week and then continue to do so once every six months as required by regulation. Licensee states she will submit a copy of the completed drill log to analyst by 05/21/24 to complete the correction.
Based on record review, the Licensee and Assistant did not comply with the section cited above in that both of their Mandated Reporter Training's has expired which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will ensure that both licensee and assistant complete AB-1207 Mandated Reporter training and send certificates to the San Diego Child Care Regional Office (SDCCRO) as proof of correction by the due date of 6/14/2024.
Based on interview and record review, the licensee did not comply with the section cited above in that the assistant, A1's immunization's were not available for review which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will ensure she receives a copy of assistant, A1's immunization's and keep a record of the required immunization's in the personnel file.
Based on interview and record review, the licensee did not comply with the section cited above in that assistant, A1's CPR card was not available for review which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will ensure assistant, A1, provides a current CPR card to keep on file.
Based on record review, the licensee did not comply with the section cited above in (3) out of (6) children did not have any forms completed for review which poses a potential health, safety or personal rights risk to persons in care.
Resolution: LPA provided Licensee the LIC311D which lists required forms for children's files. Licensee stated she will provide the three children's completed files to LPA by 6/14/24.
