Based on record review, the licensee did not comply with the section cited above in 2 out of 2 persons by not having a current Mandated Reporter Certificate which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee and helper will renew Mandated Reporter Certification on the website: www.mandatedreporterca.com. and send it to LPA via email.
Based on observation and record review, the licensee did not comply with the section cited above in 4 out of 8 children sleep logs (C1-C4) which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will comply with regulation by monitoring and keeping accurate sleep logs. Licensee will send proof of sleep logs to LPA for review from 06/07/25 - 06/13/25.
Based on record review and interview the licensee did not comply with the section cited above in that she was unaware that this form was required for children under 12 months. This poses a potential health, safety or personal rights risk to children s in care.
Resolution: Licensee states that she will have the form completed and on file by the plan of correction date and send a copy to Licensing for verification. She says that she understand the requirement and will ensure this form is on file in the future for all children under 12 months old.
