Based on LPA observation and licensee interview, the licensee did not comply with the section cited above in which the licensee is not documenting the 15-minute checks for infants under 24-months, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee was reminded she must check and document each 15-minute check for every infant under 24-months. The licensee will also submit a written declaration of her understanding of the regulation and how she will make sure to comply with the regulation.

Based on observation, the licensee did not comply with the section cited above as there were shaving cream, razor, facial lotions, shampoos and bar of soap inside medicine cabinet and bathtub, all accessible to children, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Assistant immediately removed all items from medicine cabinet and bathtub and placed them out of the reach of children. No further plan of correction needed. Deficiency was cleared during inspection.

Based on staff interviewed and record review, the licensee did not comply with the section cited above as two of the assistant present had not taken the required AB-1207 Mandated Reporter Training, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Provider stated that she will ensure that all staff complete the required Mandated Reporter Training by the due date. Provider will send a copy of certificate of completion to the San Diego Child Care Regional Office (SDCCRO) as proof of correction.
