Based on observation, the licensee did not comply with the section cited above as backyard has accessible grill and lumber, sharp and rusty chicken wire around window, paint and broken pottery in accessible side yard areas. which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states will secure two fences to make hazards in side yards inaccessible to children and provide proof to LPA no later than close of business 9/9/22.
Based on record review the licensee did not comply with the section cited above as staff 2 and staff 3 do not have current mandated reporter training certificates which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states will ensure that staf 2 and staff 3 complete mandated reporter training and provide proof to LPA no later than close of business 9/9/22. Licensee states will ensure that all staff remain current on Mandated Reporter traning in the future.
