Based on observation the licensee did not comply with the section cited above as there was a knifeblock full of knives on kitchen counter and aff limits areas were not made inaccessible and contained, perfumes and paints within reach which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee removed knives and stated will install door knob covers or locks on doors to all off limits areas and provide proof to LPA no later than close of business 4/22/22.
