Based on record review, the licensee did not comply with the section cited above. Mrs. Rivera failed to maintain the 15-minute check log on napping infants. She currently has 4 children under the age of two in care. Not maintaining nap logs poses a potential health, safety or personal rights risk to persons in care.

Resolution: LPA provided Mrs. Rivera a copy of the 15-minute infant nap log today. She stated that she conducts the 15-minute check but did not maintain the log. She will start using the nap log and submit sample copies to the department no later than end of businessd ay of 1/26/2023.

Based on record review, the licensee did not comply with the section cited above. Mrs. Rivera's and her 2 helpers have expired Mandated Reporter Training certificate. Mrs. Rivera was reminded that the training must be renewed every 2 years. Having expired Mandated Reporter Training posesa potential health, safety or personal rights risk to persons in care.
