Based on analyst record review and interview, the licensee did not comply with the section cited above as she has not conducted fire and disaster drill with children in care which poses/posed a potential health, safety or personal rights risk to them.
Resolution: Analyst provided licensee with a blank fire drill log and licensee states she will conduct fire and earthquake drills with children in care, document them and send analyst a copy of the updated log by 08/25/25 to complete the correction. Licensee will continue to conduct drills every six months as required.
Based on analyst observation and interview, the licensee did not comply with the section cited above as during today's visit she did not have a smoke detector which poses/posed a potential health, safety or personal rights risk to children in care.
Resolution: Licensee states she will purchase a new combination smoke/carbon monoxide detector and install it in the home and then send analyst a copy of the purchase receipt by 06/17/24 to complete the correction.
Based on analyst observation, the licensee did not comply with the section cited above as latches were broken for the drawer where knives are kept in the kitchen and in the cabinet underneath the sink where cleaners are kept which poses/posed a potential health, safety or personal rights risk to children in care.
Resolution: Licensee was able to replace the broken drawer latch but not the cabinet latch. Licensee purchased a cabinet latch on-line before analyst completed his vist and will install it when it arrives and send analyst a photo of it by 06/17/24 to complete the correction.
Based on observation and interview, the licensee did not comply with the section cited above as she has not conducted fire and disaster drill with children in care which poses/posed a potential health, safety or personal rights risk to children in care.

Based on analyst interview and record review, the licensee did not comply with the section cited above as she was not maintining a safe sleep for the one infant in care, child #1, which poses/posed a potential health, safety or personal rights risk to infants in care.
Resolution: Licensee was provided a sample of the safe sleep log and states she will maintain the log everyday the child naps going forward. She will aslo send analyst a copy of the log for child #1 from 06/13/24 - 06/21/24 to complete the correction and demonstrate compliance with the nap logging requirement.
Based on analyst interview and record review, neither the licensee or her assistant daughter, Alexis Sanchez complied with the section cited above as they did not have a mandated reporter certificate from the past two years which poses/posed a potential health, safety or personal rights risk to children in care.
Resolution: Licensee and assistant were provided with a mandated reporter training flyer and stated they will complete the free on-line training and send analyst a copy of their certificates of completion by 07/15/24 to complete the correction.
Based on analyst record review, the licensee did not comply with the section cited above as neither she or her assistant daughter were able to provide analyst with a current copy of their CPR/First Aid certifications which poses/posed a potential health, safety or personal rights risk to children in care.
Resolution: Licensee states she and her daughter will take an EMSA approved CPR/First Aid training class and provide analyst with copies of their completed certifications by 07/15/24 to complete the corrections.
Based on analyst record review, the licensee did not comply with the section cited above as children #1 and #5 did not contain any record files which poses/posed a potential health, safety or personal rights risk to children in care.
Resolution: Licensee states she will have the parents complete the record forms for children in care and then send analyst copies of the completed forms for child #1 by 06/24/24 to complete the correction.
Based on analyst record review, the licensee did not comply with the section cited above as she did not have a completed plan for the only infant in care, child #1, which poses/posed a potential health, safety or personal rights risk to infants in care.
Resolution: Licensee was provided with a blank copy of the infant sleeping plan form and will provide it to parent to complete and return. Licensee states she will submit a copy of the completed from to analyst by 06/24/24 to complete the correction.
Based on analyst observation and interview, the licensee did not comply with the section cited above as during today's visit she did not have a carbon monoxide detector which poses/posed a potential health, safety or personal rights risk to children in care.
Resolution: Licensee states she will purchase a new combination carbon monoxide/smoke detector and install it in the home and then send analyst a copy of the purchase receipt by 06/17/24 to complete the correction.
Based on analyst observation & interview, the licensee did not comply with the section cited above as she has not conducted fire and earthquake drills within the past six months with the day care children which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states she will conduct fire and earthquake drills with children in care and document them on a drill log and submit a copy of that log to analyst by 11/01/21.
Based on analyst interview & record review, the licensee did not comply with the section cited above as she and her daughter helper have not completed the mandate reporter training which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states she and her daughter will complete the free mandated reporter training found at www.mandatedreporterca.com and submit copies of the completion certificates to analyst by 11/22/21.
Based on analyst interview & record review, the licensee did not comply with the section cited above as she did not have copies of her immunization records available showing the mandatory immunizations which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states she will locate proof of her and her daughter's immunization records for MMR, DTaP and seasonal influenza (or a declination note) and submit a copy of the records to analyst by 11/22/21.
Based on observation, the licensee did not comply with the section cited above as seven month old infant was sleeping in one of the off limits home bedrooms which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Infant was removed from the bedroom during visit and licensee states the child will not sleep in the bedroom going forward. Bedroom is to be kept off limits with locking door knob and child will sleep in playpen in facility living room.
