Based on record review and interview, licensee and staff (S1) did not comply with the section cited above in that licensee’s Mandated Reporter Training has expired and staff (S1) did not have a certificate for review which poses a potential health, safety or personal rights risk to persons in care.
Resolution: LPA provided Licensee with written instructions on how to locate the Mandated Reporter Training online. Licensee states she and staff (S1) will complete this online course and provide SDRO with a copy of their training completion certificate no later than 11/20/2023.
Based on interview and record review, the Licensee did not comply with the section cited above as the Licensee’s CPR/First Aid certification expired on 6/2021 which poses a potential health, safety or personal rights risk to children in care.
Resolution: Licensee stated she will enroll in an EMSA CPR/First Aid training class, complete it and will submit a copy for the plan of correction to the San Diego Regional Office by 11/20/2023. Licensee will ensure to renew her certification every two years prior to the expiration date.
