PIMENTEL, KATIA FAMILY CHILD CARE

Based on observation, record review, and interview, the licensee did not comply with the section cited above in that the licensee and assistant S1 did not have the Mandated Reporter training for review which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will ensure that herself and S1 register for Mandated Reporter training. Licensee will copy and send the completed certificates to the SDRO by 4/18/25.

Based on observation, record review, and interview the licensee did not comply with the section cited above in that assistant S1 and S2's immunization records were not available for review which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will obtain a copy of S1 and S2's immunization records and submit the copy to the SDRO by 4/3/25.

Based on observation, record review, and interview,, the licensee did not comply with the section cited above in that personnel records were incomplete and missing required forms for assistant S1 and S2 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will ensure to provide S1 and S2 the required licensing forms to fill out and return. Licensee will submit the completed forms to the SDRO by 4/3/25.

Based on observation, and record review, the licensee did not comply with the section cited above as 13 children were present, none of which were school-age, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide LPA with a written plan of operation by 08/01/2022 outlning how she will maintain ratio and capacity requirements in the future.
