Based on record review, the licensee did not cpmply with the section cited above and did not have a current fire/ earthquake drill performed wich poses/posed a potential health, safety or personal right risk to persons in care.

Resolution: Licensee will complete fire/earthquake drill and will email proof to LPA by 05/29/2025.

Based on observations and record review, the licensee did not comply with the section cited above in 2 out of 2 adults not in compliance with mandated reporter certificate training which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Liensee agreed to take Mandated reporter training for her and helper. Also, Licensee will complete and keep acurate staff files and will have TPS program help her review files to ensure accuracy and compliance. Licensee will send LPA email to show proof of completed staff files after TSP visit is completed.

Based on record review, the licensee did not comply with the section cited above as Licensee did not lock up medications in kitchen and dining room, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: LIcensee states will lock all medications, vitamins and supplement in a scured place thus making them inaccessible to children in care and provide proof to LPA no later than close of business 6/10/22.

Based on record review, the licensee did not comply with the section cited above as there as last disaster drill was 5/21/21which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: LIcensee states will complete and emergency disaster plan and provide proof to LPA no later than close of business 6/10/22.

Based on record review, the licensee did not comply with the section cited above as staff #1 and staff #2 have not completed mandated reporter training as required which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: LIcensee states that she will ensure that staff #1 and staff #2 complete mandated reporter training and provide proof to LPA no later than close of business 6/10/22

Based on observation the licensee did not comply with the section cited above as Licensee stated that the area was off limits and refused LPA entrance into an accessible bedroom in which an adult male was residing which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee states will update LIC999A to reflect new off limits areas in home and will make those off limits areas inaccessible through the use of latches, locks, gates or doorknob covers and provide proof to LPA no later than close of business 5/12/22

Based on record review, the licensee did not comply with the section cited above as 10 of 10 children in care do not have complete child files which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: LIcensee states will ensure that all children in care have complete files and will provide proof to LPA no later than close of business 6/10/22.

Based on record review, the licensee did not comply with the section cited above as child roster is not current which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states will provide updated child roster and provide to LPA no later than close of business 6/10/22.

Based on record review, the licensee did not comply with the section cited above as staff #1 and staff #2 do not have complete staff files which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: LIcensee states will create complete staff files for staff #1 and staff #2 and provide proof to LPA no later than close of business 6/10/22.

Based on observation, the licensee did not comply with the section cited above as Licensee allowed child #1 to sleep on treadmill and did not provide suitable sleeping equipment to meet the child's needs which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee states will provide suitable napping equpment for child #1 and provide proof to LPA no later than close of business 5/12/22
