PETIT-LOUIS, ESTIVERNE FAMILY CHILD CARE

Based on analyst record review, the licensee did not comply with the section cited above as posted disaster drill log did not have any entries in it which poses/posed a potential health, safety or personal rights risk to children in care.

Resolution: Licensee states he will document all drills conducted with day care children including any done in the past six months and send analyst a copy of completed log by 02/09/26 to correct the deficiency.

Based on analyst record review, the licensee did not comply with the section cited above as his girlfriend assistant, Blondine Denaud, did not have any proof of immunizations on file at the facility which poses/posed a potential health, safety or personal rights risk to children in care.

Resolution: Licensee stated he is unsure if Ms. Denaud has had the required DTaP/MMR vacccinations but will check her medical provider and either send analyst a copy of proof of the immunizations, have her obtain the shots or provide documentation for potential exemption and send a copy of that to analyst by 03/02/26 to complete the correction.

Based on analyst record review, the licensee did not comply with the section cited above as child #2 did not have her immunizaations on file which poses/posed a potential health, safety or personal rights risk to children in care.

Resolution: Licensee was able to have analyst review a digital copy of the child's immunizations but is still required to transfer them to a blue PMS 286 immunization card to meet the regulation requirement. Licensee states he will transfer the shots to the blue card he has for the child and send analyst a copy by 02/09/26 to correct the deficiency.
