Based on Licensing Program Analysts Selina Siao and Daniel Pena's observation and interview with licensee, the licensee did not comply with the section cited above in 6 out of 6 of the chidlren in care that are two and older are not wearing a face mask during indoor activity time which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will let the parents know about mask mandates requirements for children two and older to wear mask at all child care facilities while indoor. Licensee understands that she will be sure that the parents drop off the child with a mask and facility will also provide mask to children in care if they forget to bring one to care.

Based on Licensing Program Anlayst Selina Siao's observation today, the licensee did not comply with the section cited above as Analyst observe a bottle of pine sol, all purpose cleaner and hairspray under the sink cabinet accessible to children as the latch is broken which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee replaced the bathroom sink cabinet latch during today's inspection. Licensee shall submit a written plan of correction on what she will do to ensure that she doesn't violate this regulation again in the future.

Based on licensee's statement, the licensee did not comply with the section cited above as her and her two helpers present today have not taken the online mandated child abuse training within the last two years which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that her and her helpers will complete the online mandated child abuse training and she will submit the certificates to Analyst upon completion.

Based on interview and record review, the licensee did not comply with the section cited above in 4 out of 4 infants in care does not have a safe sleep log available for review which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee was provided with the safe sleep log and she stated that she will start using the form to log the infants are checked every 15 minutes and will document that on the form starting today. Licensee shall submit a written plan of correction to Analyst by tomorrow.
