Based on observation, the licensee did not comply with the section cited above as there was a five gallon bucket underneath an outdoor sink in the back yard that had approximately eight inches of water on the bottom, though no children were outside at the timein which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee immediately corrected the violation as she immediately dumped the water. Licensee stated that in future she will ensure that the bucket is always emptied after use.

Based on interview and record review, the licensee did not comply with the section cited above as six of six staff do not have current mandatory reporter training which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will ensure that 6 of 6 staff complete mandatory reporter training and provide proof of completion no later than close of business 3/2/22.

Based on interview and record review, the licensee did not comply with the section cited above as 5 of 6 staff (S1, S2, S3, S4, S5, S6) do not have complete staff files which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states that she will provide LPA with complete staff files for (S2, S3, S4, S5, S6) no later than close of business 3/2/22

Based on record review, the licensee did not comply with the section cited above as 15 of 15 children do not have complete child records which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states will provide LPA with complete records for all children in care no later than close of business 3/2/22.

Based on interview and record review, the licensee did not comply with the section cited above as Licensee does not have a current child roster which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states will provide LPA with current LIC9040 no later than close of business 3/2/22.

Based on interview and record review, the licensee did not comply with the section cited above as Licensee has not completed a CPR/First Aid class since 2018 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states will complete CPR/First Aid and will provide proof of completion to LPA no later than close of business 3/2/22

Based on observation, the licensee did not comply with the section cited above as 12 of 12 children in care were not wearing masks which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee states that they will provide masks to children and provide proof of mask use by children no later that close of business 2/10/22 and that in the future Licensee will provide masks daily and ensure that all children in care are following the County mask mandate.
