Based on interview and observation, the licensee did not comply with the section cited above, Licensee did not have pool alarm which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will purchase pool alarm and send proof to Licensee by 5/8/25 via email Annette.Sutherland@dss.ca.gov or video conference by 5/8/25.

Based on interview and observation the licensee did not comply with the section cited above and did not hae a life ring for the pool which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will purchase a life ring with a minimum exterior diameter of 17 inches and labeled as approved by the United States Coast Guard. and send proof to Licensee by 5/8/25 via email Annette.Sutherland@dss.ca.gov by 5/8/25.

Based on interview and observation the licensee did not comply with the section cited above, and did not have a rescue pole with a body hook and a minimum fixed length of 12 feet which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will send proof to licensee via email to Annette.Sutherland@dss.ca.gov of  a rescue pole with a body hook and a minimum fixed length of 12 feet .

Based on record review, the licensee did not comply with the section cited above, licensee did not have a inspection pool log which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit proof of inspection pool log to LPA via email to Annette.Sutherland@dss.ca.gov

Based on observation, the licensee did not comply with the section cited above. LPA observed three cribs/play yards with tents/coverings attached to them. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee states that she will remove the tents/coverings from the play yards/cribs and send LPA a picture of the play yards/cribs without the tents/coverings via email by POC due date of 2/4/22.

Based on record review and licensee statement, the licensee did not comply with the section cited above in one out of four children to not have an emergency information card at the facility. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee states that she will have the parent of child (C1) complete the missing emergency information form and send a copy of the document to LPA via email by POC due date of 2/4/22.

Based on observation, the licensee did not comply with the section cited above. The self-closing and self latching gate did not close and latch from all closing distances. This poses an immediate health, safety or personal rights risk to persons in care.

Resolution: The licensee states that she will repair the pool gate so that it is self closing and self latching when closed from all distances. The licensee will either send a video of the gate closing and latching or a written plan as to how the gate will be repaired to meet requirements to LPA via email by POC due date of 2/2/22.
