Resolution: Licensee was provided with a blank copy of a fire and earthquake drill log and stated will document the most recent drills completed as well as conduct any required current ones and submit a copy of the updated log to analyst by 04/28/25 to correct the deficiency.

Based on analyst observation, the licensee did not comply with the section cited above as all three napping playpens had blankets and pillows in them including one with a napping infant which poses/posed a potential health, safety or personal rights risk to infants in care.

Resolution: The deficiency was corrected when licensee removed all the blankets and pillows from the infant playpens and understands there should never be any loose objects in any of the sleeping equipment of any infants at any time. Analyst also provided licensee with a copy of the infant safe sleep regulations.

Based on analyst record review, the licensee did not comply with the section cited above as napping logs for three infants in care were not being kept which poses/posed a potential health, safety or personal rights risk to infants in care.

Resolution: Licensee was provided with blank copies of safe sleep logs and stated that she will monitor and log infant's naps as required by regulation beginning tomorrow through 04/25/25 and then send copies of the completed sleep logs for all three infants to analyst by 04/28/25 to correct the deficiency. LIcensee understands also that she will continue sleep logs going forward for all children until they turn two years of age.

Resolution: The deficiency was corrected with licensee understanding that the door to the napping room must remain open at all times when infants are napping so that they may be easily observed.

Resolution: Analyst provided licensee with the a Mandated Reporter Training flyer which provides information on how and where to take the free on-line training. Licensee states she and her son will take the training and submit a certificate of completion to analyst for both of them by 05/19/25 to correct the deficiency.

Resolution: Licensee corrected the deficiency during the visit by removing the pillow and blanket from the playpen and placing a fitted sheet only on the mattress. Licensee understands that there should never be any loose objects in playpens from this point forward.

Licensee will send a copy of the completed logs for both infants for the period from 08/04/22 to 08/12/22 and submit a copy of those records to analyst by 08/15/22 to complete the correction.
