MORFIN, BELIA & ROSALBA FAMILY CHILD CARE

Based on analyst interview and record review, the licensee did not comply with the section cited above as, though Ms. Morfin stated they have been conducting fire drills, they did not have the dirlls documented as required which poses/posed a potential health, safety or personal rights risk to children in care

Resolution: Licensee was provided with a blank copy of a fire and earthquake drill log and stated will document the most recent drills completed as well as conduct any required current ones and submit a copy of the updated log to analyst by 04/28/25 to correct the deficiency.

Based on analyst observation, the licensee did not comply with the section cited above as all three napping playpens had blankets and pillows in them including one with a napping infant which poses/posed a potential health, safety or personal rights risk to infants in care.

Resolution: The deficiency was corrected when licensee removed all the blankets and pillows from the infant playpens and understands there should never be any loose objects in any of the sleeping equipment of any infants at any time. Analyst also provided licensee with a copy of the infant safe sleep regulations.

Based on analyst record review, the licensee did not comply with the section cited above as napping logs for three infants in care were not being kept which poses/posed a potential health, safety or personal rights risk to infants in care.

Resolution: Licensee was provided with blank copies of safe sleep logs and stated that she will monitor and log infant's naps as required by regulation beginning tomorrow through 04/25/25 and then send copies of the completed sleep logs for all three infants to analyst by 04/28/25 to correct the deficiency. LIcensee understands also that she will continue sleep logs going forward for all children until they turn two years of age.

Based on analyst observation the licensee did not comply with the section cited above as the door to the infant napping room was closed which poses/posed a potential health, safety or personal rights risk to infants in care.

Resolution: The deficiency was corrected with licensee understanding that the door to the napping room must remain open at all times when infants are napping so that they may be easily observed.

Based on analyst record review, the licensee did not comply with the section cited above as neither she or her helper, son Isai Quezada, had a current Mandated Reporter Training certificate which poses/posed a potential health, safety or personal rights risk to children in care.

Resolution: Analyst provided licensee with the a Mandated Reporter Training flyer which provides information on how and where to take the free on-line training. Licensee states she and her son will take the training and submit a certificate of completion to analyst for both of them by 05/19/25 to correct the deficiency.

Based on analyst record review, the licensee did not comply with the section cited above as neither she or her son had current CPR/First Aid certifications which poses/posed a potential health, safety or personal rights risk to children in care.

Resolution: Licensee states she and her son will take an EMSA approved pediatric CPR/First Aid training class and submit a copy of current certifications for both of them to analyst by 05/19/25 to correct the deficiency.

Based on analyst observation and interview, the licensee did not comply with the section cited above as previous off limits rooms were turned into the primary day care area without notifying licensing which poses/posed a potential health, safety or personal rights risk to children in care.

Resolution: Though new rooms used for the day care were inspected and found to meet regulations and to be safely childproofed, licensee states she will submit a new facility sketch that accurately depicts the rooms used by 04/21/25 to correct the deficiency.

Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above in [count] out of [total count] [(objects) (persons)] [identifiers] which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above as she has not conducted fire and disaster drills within the past six months or documented them which poses/posed a potential health, safety or personal rights risk to children in care.

Resolution: Licensee states she will conduct fire and disaster drills and then send analyst a copy of the log showing their completion to him by 08/08/22 to complete the corrections.

Based on observation, the licensee did not comply with the section cited above as she had a disallowed baby bouncer in the day care which poses/posed a potential health, safety or personal rights risk to children in care.

Resolution: Licensee corrected the deficiency today with removal of the bouncer to an off limits area of the home.

Based on analyst observation, the licensee did not comply with the section cited above as the playpen she uses for the infant in care had a blanket and pillow in it which poses/posed a potential health, safety or personal rights risk to children in care.

Resolution: Licensee corrected the deficiency during the visit by removing the pillow and blanket from the playpen and placing a fitted sheet only on the mattress. Licensee understands that there should never be any loose objects in playpens from this point forward.

Based on record review, the licensee did not comply with the section cited above as she is not keeping a safe sleep log for the two enrolled infants which poses/posed a potential health, safety or personal rights risk to children in care.

Resolution: Licensee was given a blank copy of a safe sleep log and will begin maintining sleep logs for infants in care. Licensee will send a copy of the completed logs for both infants for the period from 08/04/22 to 08/12/22 and submit a copy of those records to analyst by 08/15/22 to complete the correction.

Based on record review, the licensee did not comply with the section cited above as she and her assistant son, Isai Quezada, did not have current Mandated Reporter Certifications which poses/posed a potential health, safety or personal rights risk to children in care.

Resolution: The licensee and her assistant stated that they will complete the online Mandated Reporter Traning at www.mandatedreporterca.com and submit copies of their completion certificates to analyst by 09/05/22 to complete the correction.

Based on observation, analyst found that the licensee did not comply with the section cited above as a new home was constructed in the back yard of current day care home which poses/posed a potential health, safety or personal rights risk to children in care.

Resolution: Licensee states she will submit a new facility sketch depicting the new home on an updated yard sketch and also submit a statement to analyst on how they will make the day care yard temporarily and permanently inaccessible to the day care children by 08/08/22.

Based on analyst observation and record review, the licensee did not comply with the section cited above as the previously off limits first bedroom was made into a for use day care room and the previous day care playroom is now only a pass through room to outdoors which poses/posed a potential health, safety or personal rights risk to chldren in care.

Resolution: Licensee states she will submit a new facility sketch that shows all the for use and off limits areas to analyst by 08/08/22 to complete the correction.

Based on record review, the licensee did not comply with the section cited above as children #1-3 and #7-10 did not have immunization records in their files which poses/posed a potential health, safety or personal rights risk to children in care.

Resolution: Licensee contacted parents and had them bring records for all children who needed them and transferred them to blue cards during visit to complete the correction

Based on record review, the licensee did not comply with the section cited above as she did not have a roster of children in care on file which poses/posed a potential health, safety or personal rights risk to chldren in care.

Resolution: Licensee corrected the deficiency today by completing a roster during the visit.

Based on record review, the licensee did not comply with the section cited above as the one infant under 12 months of age that she has in care does not have an Individual Infant Sleeping plan completed for him which poses/posed a potential health, safety or personal rights risk to the child in care.

Resolution: Analyst printed out and provided licensee with a blank Individual Infant Sleeping Plan for today. Licensee states that she will have the parent complete the form and send analyst a copy of it by 08/08/22 to complete the correction.
