Based on interview and record review , the licensee did not comply with the section cited above in the Licensee does not have current facility childrens roster which poses/posed a potential health, safety or personal rights ris k to persons in care.

Resolution: Licensee to make corrections.

Based on observation, the licensee did not comply with the section cited in that LPA observed food droppings that attracted ants in bedroom 2 (nap room) which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will make corrections. Licensee will clean and remove ant and ensure food droppings are cleaned up.

Based on observation, the licensee did not comply with the section cited above the wall heater does not have a safety screen in front of it which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will make corrections. Licensee will put up safety screen and/or get SDGE documentation stating wall heater is working.

Based on interview and record review, the licensee did not comply with the section cited above in that Licensee has not performed emergency drills in the last 6 months which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee to correct. Licensee will conduct emergency drill and submit to CCLD by POC date of 12/9/24

Based on interview and record review, the licensee did not comply with the section cited above in Licensee isn't maintaining sleep logs for the child under 2 years which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee to make corrections. Licensee will maintain sleep logs from 11/25/2024 moving forward and submit sleep logs to CCLD by POC date of 12/9/24

Based on observation and interview, the licensee did not comply with the section cited above in the Licensee was using the garage as additional playspace without prior approval from licensing. The garage was listed as "off limits" at the last annual which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee to make corrections. Licensee will no use garage until boxes are removed and safe for the use of children.

Based on record review, the licensee did not comply with the section cited above in three out of five enrolled children did not have proof of immunization on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee to make corrections. Ensure all enrolled childrens records will be include immunization records.
