Based on LPA observation and interview, staff Sukhri Abdi was observed supervising 6 children alone, when the LPA arrived. Ms. Abdi identified herself as an aide and no qualified teacher was present at the facility, which poses a potential health, safety or personal rights risk to children in care.

Based on records review, 14 of 14 children enrolled in the facility did not have any required children's documents/records which poses a potential health, safety or personal rights risk to persons in care.
Based on interview, the licensee did not comply with the section cited above, as they did not furnish to the Department a report of a Director change at the facility, which poses a potential health, safety or personal rights risk to persons in care.
Based on LPA observation, the licensee did not comply with the section cited above, as in the kitchen, several bottles of cleaning supplies were sharing a storage crate with canned goods and cooking oils, which poses a potential health, safety or personal rights risk to persons in care.

§1596.8662 (b) (1) Availability of information regarding detecting and reporting child abuse and neglect; training for mandated reporter who is licensed day care provider, administrator, or employee of a licensed child day care facility; proof of completion (b) (1) On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training.

(16) Pesticides and other similar toxic substances shall not be stored in food storerooms, kitchen areas, food-preparation areas, or areas where kitchen equipment or utensils are stored.
(1) The center shall give each authorized representative a copy of the Personal Rights form (LIC 613A [9/96]).
(2) Each person specified in (g) above shall have a health-screening report signed by the person performing the screening. This report shall indicate the following:
