Based on observation, the licensee did not comply with the section cited above in that the licensee did not have a carbon monoxide present in the home which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that she will obtain a carbon monoxide detector no later than 12/1/2023 and submit a copy of store receipt and photo of the installed carbon monoxide detector.
Based on record review, the licensee did not comply with the section cited above in that she did not maintain personnel record for staff Erika Ortiz which poses a potential health, safety or personal rights risk to persons in care.
Resolution: LPA provided a packet of employee form for helper's completion. Mrs. McKean will submit copy of the packet to the department no later than 12/07/2023.
Based on record review, the licensee did not comply with the section cited above in that she did not maintain a signed LIC 9052 for staff Erika Ortiz which poses a potential health, safety or personal rights risk to persons in care.
Resolution: LPA provided a packet of employee form for helper's completion. Mrs. McKean will submit copy of the packet to the department no later than 12/07/2023.
Based on interview and record review, the licensee did not comply with the section cited above in that she did not maintain the required immunization record for helper Erika Ortiz which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Mrs. McKean shall obtain a copy of Erika Ortiz's immunization record for Pertussis, Measles and Influenza and submit a copy to the department no later than 12/7/2023.
Based on interview and record review, the licensee did not comply with the section cited above in that she did not maintain immunization record on fomr PM 286 which poses a potential health, safety or personal rights risk to persons in care.
Resolution: LPA provided Mrs. McKean form PM 286. Mrs. McKean stated that she will complete the required form and submit copies to the department no later than 12/7/2023.
Based on interview and record review, the licensee did not comply with the section cited above in one infant in care. Mrs. McKean did not maintain the 15-minute nap documentation on one infant in care which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Mrs. McKean stated that she will start documenting the 15-minute nap checks starting today.
