Based on interview and record review, the licensee did not comply with the section cited above, as she did not possess and maintain the safe sleep logs for 4 of 4 infants in care, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: LPA reviewed the requirement of the Safe Sleep Log with the licensee, who stated she will begin maintaining the log immediately. Licensee will submit sample of sleep documentation to LPA by 4/28/25 and keep updated in the future. Email to Gerald.Poindexter@dss.ca.gov.
Based on observation and interview, infants C1 and C2 bottles were not labeled with their names, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that she will label all infant bottles in the future and will provide photo evidence to LPA Poindexter via email by 4/28/25 at Gerald.Poindexter@dss.ca.gov
Based on observation, LPA observed toys in the cribs of 2 of 3 sleeping infants in care, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Correction was made during visit. Licensee removed toys in the crib. LPA reviewed safe sleep regulation with Licensee.
Based on record review, 1 of 8 children, child C3, did not have immunization record in their file, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that she will obtain a copy of the child’s immunization record or a signed/dated medical exemption from a licensed physician that identifies the child’s medical condition, and the medical risk posed by immunizations. These documents will be emailed to LPA Poindexter by 4/28/25 at Gerald.Poindexter@dss.ca.go
Based on observation, the Playroom, Family Room and Kitchen contained accessible prescription medications, household cleaners, and sharp kitchen objects, as well as several electrical outlets were left uncovered, which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee removed and safely stored all the items during the inspection. This deficiency has been cleared. LPA reminded licensee that all hazardous items should be stored inaccessible to children. LPA reminded licensee that a signed Acknowledgement of Receipt of Licensing Report (LIC 9224) must be placed in the child's file for verification.
Based on interview and record review, the licensee did not comply with the section cited above as licensee and her husband last completed the online mandated child abuse training in January 2020 which is more than the required every two years this poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that her and her husband will renew their online mandated child abuse training and will submit the training certificate to Licensing no later than 10/31/2022.
Based on observation the licensee did not comply with the section cited above as facility was out of ratio as licensee was alone supervising 8 children during nap time including 3 infants and 5 preschool age children until her husband returned from the store which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that she will make sure that her husband do not go out to the store unless she is operating in the small license capacity. Licensee shall submit a written plan of correction to LPA no later than 10/17/2022.
Based on interview with licensee and record review, the licensee did not comply with the section cited above in as licensee has not conduct a fire and disaster drill with the children since /15/2021 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that she will conduct a fire drill with the children and she will take a picture of her drill log and submit it to Licensing no later than 10/17/2022.
