Based on analyst record review, the licensee did not comply with the section cited above as children #1 & #2 did not have any immunization records on file and children #3 & #5 did not have their immunizations transferred to blue shot cards as required which poses/posed a potential health, safety or personal rights risk to children in care.
Resolution: Analyst provided licensee with blue immunization cards and licensee stated she will either obtain the missing immunization records from parents and transfer the shot records to them or transfer the existing shot records on file for the children in the alternate situation and then send analyst a copy of the completed blue card for a child of her choice from the two groups (a total of two) by 07/14/25 to correct the deficiency.
