Resolution: Mrs. Machado will obtain the required Pertussis immunization record for staff Catherine Tovar Franco and submit a copy to the department no later than 10/23/23.
Based on interview and record review, the licensee did not comply with the section cited above. Mrs. Machado did not have a sleeping plan for infant M.G. on file. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: LPA provided Mrs. Machado a copy of the sleeping plan. She will have M.G.'s mother complete the form and submit a copy to the department no later than 10/23/23.
